    Jennifer Finney Boylan spent her first 40 years as a woman in a man’s body. She’s Not There : A Life in Two Genders is Jennifer’s story about growing up as James, knowing with “heartbroken conviction” that she was living in the wrong life, and hoping to be “cured by love.”

    James Boylan met and married Grace and became a professor at Colby College in Maine. Together they raised two sons. Boylan’s life included playing in various bands, writing and publishing successful books, and constant excruciating dissonance and concealment. At age forty-two, after years of therapy and exploring transgender issues, Boylan began the transition to a female body. This was achieved through hormone therapy and eventual gender reassignment surgery.

    We are led to understand that she didn’t DECIDE to make this transition, but rather stopped resisting it. Boylan’s writing is often wry and even funny, but she never tries to conceal the pain of her transgendered life and the redemptive integration of her reassignment. She moves back and forth in time, framing the story with an emotionally revealing opening and then varying the intensity throughout.

    The impact on Boylan’s family and friends was “atomic.” Boylan chose fictional names for the non-public people in this memoir, and her choice of the name Grace for her spouse cannot be by chance. After years of marriage, Grace mourned the loss of her husband while finding enough love and faith in her life partner to stay in the relationship. Unstinting in her own self-exposure, Boylan leaves a veil of privacy over the nature of her spouse’s accommodation to this change.

    In a riveting afterword (from which my title line is taken), Boylan’s closest friend, author Richard Russo, explores his own response to Jennifer’s transition and its impact. Russo writes of Grace: “Years earlier, her heart had inclined in the direction of another soul…We love whom we love. In the past two years, for Grace, everything had changed and nothing had changed. Her heart still inclined, as was its habit.”

    She’s Not There : A Life in Two Genders is a beautifully written story of hope, inviting us to reassess the meaning of love and friendship. To call it a book about sex change would be to sell its message far too short.

    Because the nerves and muscles connected to the tailbone run throughout the lower body and up the spine, the effects of coccyx injuries can be far reaching:

    • headaches
    • low back pain
    • sciatica pain
    • pain in the coccyx area that does not abate over time (coccygodynia or coccydynia)
    • chronic digestive problems, especially constipation
    • menstrual cramps
    • pain during intercourse (for women)

    Some of these problems may appear right after a tailbone injury, while others show up in the following months or years. Even if am injury appears to be healing on its own, it's usually a good idea to investigate treatment (see below) to realign the lower spine.

    Treatments and Solutions for Coccyx Injuries and Pain

    Most doctors recommend that pain be treated with ice, pain killers, and by avoiding further trauma to the area using special coccyx cushions that avoid placing pressure on the tailbone, so that coccydynia can mend itself.

    If coccyx pain (and its associated problems) does not go away within a few weeks, the only medical option is surgery to remove the coccyx – which does not always work to alleviate the pain (prompting some doctors to consider lasting coccyx pain a psychosomatic issue instead of a physical pain condition).

    A more holistic approach links many symptoms (see above) with tailbone injuries. These problems are best addressed by healing the source of the problem – the original coccyx misalignment.

    Coccyx injuries can be treated on a deep level through practices like reiki, energy medicine, and acupuncture, but the most effective treatments for this kind of injury are craniosacral massage and osteopathy. These practices correct the subtle misalignment of the coccyx that causes digestive problems, cramps, and low back pain – bringing relief for pain and any other symptoms.

    Coccygectomy Surgery – Complications

    Coccygectomy surgery is done for removing the tailbone or coccyx for patients suffering from coccyx pain. Surgery is reserved for cancer or malignancy tumors or whose coccyx fails to respond to the nonsurgical treatments like, use of seat cushions, medications and local injections given under the guidance of fluoroscope.

    In human beings, coccygectomy is last treatment option for coccydynia, however considered as the necessary treatment option for sacrococcygeal teratoma and also for other tumors associated with germ cells that arise from tailbone.

    COMPLICATIONS WITH COCCYGECTOMY SURGERY:

    The most common complication with coccygectomy is the infection at surgical portion. Infections are common after the surgeries but this is due to surgery site is closer to the anus and colon. Hence, leads to contamination of bacteria at the site of surgery, by the bacteria from patient’s stool, that is bowel movements. Coccygectomy might result in repeated surgery at the same site for treating the infection. This infection is seen in nearly 20% of patients who undergone coccygectomy.

    Another common complication is the continuous pain at surgical site and also fails to relieve coccyx pain which patient was suffering prior to the surgery.

    For preserving normal defecation, normally coccygectomy accompanies with re-attachment (this is also called “re-approximation”) of perineum and two levator ani muscles. These are the parts of pelvic floor.

    The rare complication in adults is that who undergo this surgery is subsequent to coccygeal or perineal hernia. Bowel and other pelvic organs bulge downwards and also out of pelvis from the weakened pelvic floor. This is not reported by the persons who had coccygectomy as a child or baby. Mild version of hernia prolapses from pelvic floor.
